City of Baker giving out pre-registration packet for COVID-19 testing

There’s a one day opportunity for Baker residents to sign up to get tested and it’s open for everyone.

Packets will be available for pickup Tuesday at Baker City Hall at 9 a.m. for any anyone regardless of their age.

Quantities are limited to around 150 packets, so they’ll be handed out on a first come, first serve basis.

Drivers license & insurance card will be required both the day of testing and a copy must be included in your packet when you return it.

You must return your packet no later than 12:00 p.m. Thursday, May 21, 2020 along with a copy of your drivers license & health insurance card in order to pre-register. Packets can be sent back by:

  • Scan and email to awilliams@cityofbakerla.com
  • Drop off your copies at the front door where you picked your packet up

Testing will be conducted the following week on Tuesday, May 26th at Advantage Charter School located at 14740 Plank Road.

There will be no on site registration on the day of testing and if you do not pre-register you will not be tested.
